What to Bring to Your Appointment
- Your insurance card
- Physician referral forms if required by your insurance provider
- A list of current medications, including the dose and frequency
- Pertinent information about your medical and surgical history
- Any recent x-rays or other test results related to your condition
First Appointment
When you arrive for your first appointment, you will receive a thorough evaluation of your health history and a physical. Once we obtain this initial information, we will work with you to develop a comprehensive treatment plan and provide you with the information you need to make sometimes difficult treatment decisions. Our staff is committed to individualized patient care and discretion in all patient concerns.
Insurance and Payment Information
Dr. Sadeghi-Nejad participates with a number of insurance companies listed below.
If you are not insured with a participating company, but have PPO or "out of network" access, our billing office will simply bill the company on your behalf (so you will not have to do any extensive paperwork) soon after your visit. Our experience has shown that most companies will process your payment within 2-3 weeks.
If you do not have health insurance or have an HMO policy that does not allow any out-of network visits, various payment plans are available and can be arranged through the billing office. Please ask at the time of your visit.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work with patients who require a payment plan.
